Application Information

On April 8, 2007 our family gained an angel!  Alyssa Rose was born still just four days before her scheduled c-section appearance.  We were devastated and confused, but instantly knew that her memory needed to live on in children somehow!  We decided to start a foundation with the sole purpose of helping other children in need.  As a developmental interventionist and teacher, my husband and I see daily how hard it is for families of children with special needs to meet all the demands, especially financially.  There always seems to be that piece of equipment that would help a family, but funds are not there.  There is always that extra therapy, such as horseback riding that would help, but again funds are not there.  We would like to help.  It is our mission to provide funds for the equipment/extra therapy.  We continue to raise money through events and activities to assist with this. 

If you have a child between the ages of newborn – 7 and there is a piece of equipment or a therapy service that would help your child, but you just can’t afford it, please fill out the attached application.  We will look at each story and need, but please understand that we are unable to help everyone.  A team of therapists (OT, PT, and SLP) has been formed to determine the “winner”.  Please remember no piece of equipment is too small to request, even if it is a therapy ball, we are willing to accept your application.

A completed application should include;

• A completed form (attached)

• A letter from a therapist/doctor or educational professional with details about the equipment/service and how it will help your child

• A picture and ordering information of the equipment

• If requesting an extra therapy service, contact information on the facility offering the service

APPLICATION DEADLINE IS NOW SEPTEMBER 20, 2008!!!!!!!
